Ask HN: How to workaround inaccurate health insurance

After circumstances lead to a total of 9 different insurance plans within three years I've noticed a pattern: coverage and network information is often inaccurate. This has led to situations like flu shots not being covered since pharmacies and the HMO give different answers, double billing, outdated policy being used (despite giving correct card every time), finding out doctor or service isn't covered after all (we often check first before seeking care), and strange discount games where the billed amount plummets when challenged over the phone.

Anyone else run into complications like these or have any solutions?
